Acute coronary obstruction is a rare but a major complication of the TAVI procedure. Given its overwhelming risks, as well as the frequent need to re-engage coronary ostia as the TAVI indication expands towards younger, lower-risk patients, an appropriate strategy for protection and re-access of coronary ostia after TAVI is needed.
